Email is supposed to include an "In-Reply-To:" header that should be used
to build the thread tree. The subject-line should not be necessary for
threading.

I have seen people doing all sorts of weird things, so it might be that
the person is creating a new message with a manually typed subject
(perhaps pasting in any previous text) or manually editing the subject
after clicking the 'Reply' button.
