Email is supposed to include an "In-Reply-To:" header that should be used
to build the thread tree. The subject-line should not be necessary for
threading.
Correct. I have correspondents in other countries with different prefixes
for "Re:" yet the emails thread just fine.

It could be Apple Mail. I have one person who uses Apple Mail and their
responses always come as an attachment and often not in the thread.
I use Apple Mail and it works just fine.
