 The Twelfth Doctor. "Hell Bent" is the episode where 'The
General' regenerated and changed gender whilst doing so, and it
was seen on-screen after the 12th Doctor shot him/her... as we
were talking about here only yesterday. (You do read RADW,
right?)

That was all hallucinated. What seemed to be happening is that the general who Capaldi murdered had prior to that event been forcibly regenerated into a man in order to fight in the Time War because the Time Lords considered that female soldiers were inferior and she resented that and made it known to everyone after she returned to female form.
