The episode aired in 1986 and I haven't watched it since I'm guessing around 1987, so my memory may be a little fuzzy But here's what happened:
In Mindwarp (part of The Trial of a Time Lord), Peri (Nicola Bryant) was written out. Her character had a tragic end when an evil alien replaced her brain. The king then shows up and kills her. A later episode, "The Ultimate Foe," retconned that to say it was all a trick and she was really living with the king.

Then in the next episode the Doctor has a new companion (Mel). And as far as I can recall, at least in the episodes I've watched, this was the first time he had a new companion without that companion being properly introduced. Then later they reton Peri is not actually dead, but completely gloss over that means the Doctor left her. It's a just a throw away line. It was a trick, she's not dead, moving on.
But the bigger question is, how in the world do I still remember all of this?!?
