It's known that Wilson was so sick that there were times he was
incapable of making decisions. This was hidden from pretty much
everybody.

That's clearly wrong then in a moral sense. Was it illegal though? Isn't
it the American standard that everything's legal unless it is codified
in law that it is illegal? Was there an actual law against his wife and
doctor making decisions on his behalf?

The president has specific duties in the Constitution and in law which
others cannot be designated to carry out. If Wilson didn't sign
legislation, is it statutory law? The president is the highest civilian
in charge of the military. Can commands be lawfully issued? It's pretty
damn serious.

If they were to be prosecuted, I'm sure there's a way to do it but I
don't know what criminal law would have been used.

Anyway, what do you think of my proposed Amendment mandating every
Presidential/VP candidate having a full physical and mental evaluation
before being allowed to stand and then having annual re-exams while in
office? What objection could anyone possibly have to that? I would think
most voters would be happy to know that their candidates were at least
physically and mentally fit.

I have no confidence in doctors to rule on either. We're talking about
elected officials, especially the United States Senate, accumulating
power over decades who are generally older than, say, corporate
executives. Their health won't be great.

FDR had polio. No doctor would have certified him fit for office even in
1933.
