We used the large milk bags you're describing in the cafeteria of the student residence. You couldn't really spill those because the spout was sealed until after you'd installed the bag in the machine; then you cut off the tip of the spout and the handle regulated things from there on.
Of course, you *could* just lift the handle without a glass beneath the spout and then you'd get a spill.
As for milk bags at home, they only have a capacity of one litre (very close to a quart) so you won't spill all that much even if you spill a whole bag worth.
