Options seem to be:
 1. France-Spain interconnect failure
2. Cyber attack
3. Human Error
4. Exceptionally "Rare" atmospheric phenomena
5. Flying pigs/unicorns [delete as appropriate]
6. Other

7. Latent system design flaw (that precipitated, not propagated)
It seems there are two distinct questions to ask about the event:
- what was the "trigger"
- what caused the "chaotic" failure
Until those are nailed down, you can't ask the more important question:
- how to ensure stability/resiliency going forward
