"Between 12th and final" seemed definitive at the time. But what number lies
between 12 and ... say ... 5,000,000? Quite a fee. And the Valeyard was not
an incarnation of the Doctor - he was formed as "an amalgamation of your
darker sides".
We have already seen such an entity - the Dream Lord, in the episode "Amy's
Choice". Which was an eleventh Doctor episode, and the eleventh Doctor knew
that he was at least the 12th, if not the 13th (depending on how deeply he
kept his memories of the War incarnation buried).
I don't think we need to look any further than that.
