If I remember correctly, (many of) the spaghetti westerns were actually filmed in Spain, not Italy.
IMDB confirms that Fistful of Dollars was indeed filmed in Spain and goes on to list over 100 other films, some quite famous, that were filmed there, including the entire Dollars Trilogy, Cleopatra, Lawrence of Arabia, etc.:
