    I've often wondered why it has not been argued that the 2nd amendment
  clause about 'a well ordered militia' hasn't been used to mandate
  effective firearm training. After all isn't drill and training
  primarily what actual militias DO?

 If we didn't have so many examples of what happens when we allow that
millimeter, you might have a point.
 Long experience has taught us that with the gun-grabbing fundies, there is no
point where they will ever say, "All right, that's enough gun control. We
don't need any more than that."
 There's a very determined segment of our government, at all levels, that wants
to do exactly that-- and *will* do exactly that-- given the opportunity, which
accounts for what the anti-gun zealots deride as irrational intransigence
against "common sense" gun control.
 Just look what happened in New Orleans in the aftermath of Hurricane Katrina
for all the real-world evidence necessary to prove the point.
