Insurance makes doctors look sympathetic despite malpractice and just
getting things wrong. I heard a report this morning that AMA's survey of
doctors said that they spend two hours on insurance paperwork for every
hour of patient care. If a claim is denied, it can take 12 hours.

An example: A woman had a tumor in a sinus that required surgery.
Insurance denied pre-authorization because she wasn't prescribed an
antibiotic. Now, that might have been treatment if she had an infection,
but a tumor is not an infection and this one required surgery.
