up to a driveway  with a car, nose out apparently trying to drive onto
the road,  don't you watch closely what that car is going to do?

No, nobody expects every cyclist on that facility to "come to a
complete stop and wave to the driver to get his attention?" What seems
reasonable to me is that cyclists, pedestrians, joggers,people in
wheelchairs, etc should watch for drivers who are getting ready to
pull out of side steeets and driveways.

might reduce the accident rate but it certainly makes the cycle lane
unusable for commuting.

I didn't say anything about slowing down. I simply suggested watching
out for dangerous situations and reacting accordingly. A car getting
ready to pull out in front of a bicyclist from a side street or
driveway could be that kind of dangerous situation regardless of which
way the bicyclist is going or on which surface the cyclist is on.
