Apparently the treatment didn't work.  Here you claim you had "about
three car accidents".  Yet previously, it was "4 car wrecks":

I guess the accidents weren't very memorable.

Also, you had two ARRESTS.  You didn't "go down to the police
department".  Instead, you failed to appear for the initial
arraignment and probably had to be dragged into the court room for the
second appearance.

I can't tell from the information provided what actually happened.
Your resume shows that you were not working at the time (2010 - 2012)
so my guess is you got probation for the no-show but no fine.  In
other words, you weren't being judged on the merits of your symptoms
but rather on your inability to appear as promised.  Note that there
was a 522 day delay between the two arrests.  It's possible that a
generous and sympathetic judge dropped the DUI charges because you
were being treated for TBI (traumatic brain injury), but I really
don't believe that any judge would allow you continue driving if it
could eventually result in 4 car wrecks in 2 years.
