Cast fork crown failure

1950s British made, resprayed here in 2011.

Failed at walking speed, coasting to a stop sign after a ride. No rider injury.

Rider reported two weeks earlier inability to center wheel in fork, 'unusual handling', 'pulls to one side'.  Was planning to ship fork here for alignment check.
