 Individuals who underwent gender-affirming surgery had a 12.12-fold
higher suicide attempt risk than those who did not (3.47% vs. 0.29%,
RR 95% CI 9.20-15.96, p < 0.0001).

Individual who underwent gender-affirming surgery clearly suffered more severely from gender dysphoria than those who did not. This is an apples and pears comparison. I don't know how you'd quantify the severity of gender dysphoria, but any study that compared suicide risk would have to find some proxy for it to produce results that would mean what you want them to mean.
