This has been so for decades now in this country. There are very few hospitals in USA that are run on charity money. These rare hospitals are much safer, but also do not go beyond providing the minimum that can be done for a patient. So treatments in there aren't as thorough as in private hospitals, but the patient is never killed by them if health insurance or the pharma ask them to. Such hospitals have their own money to pay the cost. They receive them as charity.

So most of these emigrants are the middle class Americans.

A huge concentration camp doesn't have much need for R&D, does it, and this fact is beginning to show. In WWII Nazi party made huge money off of large industries, using the labor provided by hundreds of concentration camps' detainees.
These concentration camps, each, had a genuinely working doctor and hospital in them to do just as what private hospitals in USA do today. The doctor's job was to determine whether the sick man can recover cheaply enough and soon enough to go back to work, and otherwise get labelled to be gassed.
The situation here in USA is going that way! And the reason is, Nazis are running this country. It all started with Reagan.
